The Proceedings of the 21st Brazilian Symposium on Computing Applied to Health (SBCAS 2021) bring the articles selected and presented in the online edition of the event from June 15 to June 18, 2021. In this issue, the proceedings include 35 full articles and nine short articles. These works were selected through a double-blind peer-review process, from a total of 94 full articles and 24 short articles submitted, which implied an acceptance rate of 37.23% and 37.50%, respectively. Rodrigo de Melo Souza Veras (UFPI) coordinated the event's program committee, who organized this volume together with Lucas Ferrari de Oliveira (UFPR), the event's general coordinator. The articles that make up this volume were submitted until April 19, 2021, accepted for publication on May 17, 2021, with the final version submitted on May 28, 2021.
